Fast-track kardiyak cerrahide remifentanil ve fentanil'in postoperatif ağrı ve derlenme üzerine etkileri
Effects of remifentanil and fentanyl on postoperative pain and recovery in fast – tract cardiac surgery

AMAÇ: Çalışmamızda fast- track kardiak cerrahi planlanan hastalarda, hasta kontrollü analjezi yöntemiyle fentanil bolus ve remifentanil infüzyon uygulamasının postoperatif ağrı ve derlenme üzerine etkilerinin karşılaştırılması amaçlandı.
YÖNTEMLER: Sevofluran ve remifentanil anestezisi uygulanan, fast-track koroner baypass cerrahisi geçirecek ASA II grubu 42 hasta çalışmaya dahil edildi. Hastalar rastgele iki gruba ayrıldı. Grup 1 (remifentanil grup), 0.1 µg kg-1 min-1 postoperatif hasta kontrollü analjezi cihazı ile remifentanil infüzyon verilen grup. Grup 2, (fentanil grup ) cerrahi bitiminden 5 dakika önce 1 µg kg-1 fentanil bolus verilen grup. Fentanil bolus uygulamasından sonra fentanil bolus protokolü 0.1 µgkg-1 bolus doz ve 8 saat kilit süresi olacak şekilde düzenlendi. Postoperatif ilk 4 saatte her 15 dk. da, 12. ve 24. saatte ağrı skorları, sedasyon skorları ve ek analjezik ihtiyaçları kaydedildi.
BULGULAR: Analjezik etkinlik açısından iki grup arasında fark yoktu. Grup 1 de sedasyon seviyesi ilk 2 saat içinde daha düşük bulundu. 2. saatten sonra sedasyon seviyeleri iki grup arasında benzerdi.
SONUÇ: Fast-trak kardik cerrahi geçiren hastaların derlenme döneminde postopratif ağrıyı önlemede analjezik dozda remifentanil kullanılabilir.
OBJECTIVE: The effectiveness of remifentanil infusion and fentanyl bolus provided by the patient-controlled analgesia on postoperative pain and recovery of the patients undergoing pre-planned fast-tract cardiac surgery was studied.
METHODS: The anaesthesia including sevofluran and remifentanil were applied to 42, ASA II group patients undergoing fast –track coronary arter bypass graft surgery. Patients were divided into two groups. In Group 1 (remifentanil group), 0.1 µg kg-1 min-1 remifentanil infusion was given postoperatively by patient controlled analgesia device. In Group 2, fentanyl was administered at 10 µg kg-1 loading dose 5 minutes before the end of the surgery. After fentanyl administration, fentanyl bolus protocol was started at 0.1 µgkg-1 bolus dose with 8 minutes lockout time. Pain scores, sedation levels and additional analgesic requirements were recorded for the first 4 hours period in every 15 minutes and then 12th. and 24 th. hours postoperatively.
RESULTS: There was no difference between the groups at the analgesic effectiveness. The sedation levels were lower postoperatively first two hours in Group 1. After two hours, sedation levels were similar in two groups.
CONCLUSION: Remifentanil can be used at analgesic doses in recovery of patients undergoing fast-tract cardiac surgery.

Atan Kalpte Koroner Arter Baypas Cerrahisinde Esmololün Kardiyak Hemodinami ve Total Remifentanil Dozu Üzerindeki Etkisi
The Effects of Esmolol on Hemodynamic Parameters and Total Remifentanil Consumption in Off-Pump CABG Patients

AMAÇ: Bu çalışmada atan kalpte koroner arter baypas cerrahisi hastalarında esmololün hemodinamik etkilerinin araştırılması amaçlandı.
YÖNTEMLER: 34 ASA 2-3 grubu atan kalpte koroner arter baypas cerrahisi geçirecek adult hasta basit randomizasyonla remifentanil+sevofluran veya remifentanil+sevoflurana ek esmolol infüzyonu alacak gruplara ayrıldı. Hastaların hemodinamik parametreleri (arter kan basıncı, kalp atım hızı, kardiyak output, strok volüm)ve total remifentanil kullanımları karşılaştırıldı.
BULGULAR: Esmolol grubunda ameliyat süresince daha iyi hemodinamik kontrol sağlandı.
SONUÇ: Sonuç olarak esmolol koroner arter baypas cerrahisi hastalarında hemodinamik kontrol için anesteziklere eklenebilir.
OBJECTIVE: This study was designed to study the hemodynamic effects of esmolol infusion in off-pump CABG patients.
METHODS: 34 ASA 2-3 adult patients scheduled for elective off-pump CABG surgery were randomly allocated to receive remifentanil+sevoflurane or remifentanil+sevoflurane with esmolol infusion. Patients were compared for hemodynamic changes (arterial blood pressure, heart rate, cardiac output, stroke volume), and total remifentanil consumption.
RESULTS: The esmolol group had a better control of hemodynamics during all operation period.
CONCLUSION: We concluded that esmolol would administer to anesthetics for better hemodynamic control in CABG patients.

Epidural anestezi altında periferik damar cerrahisinde deksmedetomidin sedasyonunun miyokard iskemisi üzerine etkisi

AMAÇ: Bir alfa2 agonsit olan deksmedetomidin (deks) perioperatif süreçte sedasyon amacı ile kalp hızı kontrolündeki etkinliği ile tercih edilmektedir. Bu çalışmada epidural anesteziye eklenen deks sedasyonunun damar cerrahisinde, miyokard iskemisi üzerine etkisinin araştırılması hedeflenmiştir.
YÖNTEMLER: Epidural anestezi ile vasküler cerrahi yapılacak 48 hasta çalışmaya dâhil edildi. İki gruba ayrılan hastalardan ilk grubun (GD) sedasyonu cerrahi esnasında ve postoperatif 24 saat boyunca deks ile sağlanırken, ikinci grupta (GM) aynı amaçla midazolam kullanıldı. İki grupta da sedasyon skoru hedefine göre doz ayarları yapıldı. Epidural anestezi ve postoperatif analjezi protokolü tüm hastalar için benzerdi.
Çalışmada hemodinamik parametreler ile EKG analizi cerrahi sırasında ve postoperatif 48 saat boyunca izlendi. Troponin düzeyleri işlem öncesi, postoperatif 4, 8 24, 36 ve 48. saatlerde ölçüldü. Ağrı değerlendirmesi vizüel analog skala (VAS) ile yapıldı.
BULGULAR: Kalp hızı GD’de tüm zamanlarda GM’den düşük bulundu. Meydana gelen iskemik epizodlar GM’de GD’ye göre anlamlı olarak daha uzun sürmüştür. Troponin seviyesi 24, 36 ve 48 saatlerde GD’de anlamlı olarak daha düşük olduğu görüldü (p<0.05). Postoperatif analjezik tüketimi karşılaştırıldığında, 24 ve 48.saatlerde GD’de anlamlı olarak daha az tüketim söz konusu oldu (p<0.01).
SONUÇ: Epidural anestezi ile deks, midazolam gibi yeterli sedasyonu sağlamıştır. Üstelik deks infüzyonu altında kalp hızı ve troponin düzeyleri daha düşük seyretmiştir. Yine deks infüzyonunda analjezik gereksinimi de azalmıştır. Bu etkiler infüzyon kesildiği halde devam etmiştir.
OBJECTIVE: Dexmedetomidine (dex) has beneficial effects on heart rate and provides adequate sedation in the perioperative period. We aimed to investigate the effects of dex added to epidural anaesthesia on myocardial ischemia and postoperative analgesic requirements in peripheral vascular surgery.
METHODS: During surgery under epidural anaesthesia, patients were allocated in 2 groups. In group D, sedation was achieved with dex during surgery and in ICU for the first postoperative 24 hours. The second group (GM) received midazolam sedation for the time. Both drug infusions were titrated to achieve a target level of sedation. Epidural anaesthesia and postoperative analgesia were identical for two groups.
Hemodynamic parameters and ECG analysis were recorded during surgery and in ICU for 48 hours. Troponin levels were measured at the beginning and postoperative 4th, 8th, 24th, 36th, 48th hours. Visual analogue scale (VAS) and postoperative analgesic requirements were also noted.

RESULTS: In GD, heart rate was slower than GM at all times. Duration of ischemic episodes were significantly shorter in GD than GM). Troponin levels were found to be significantly lower in GD at postoperative 24th, 36th, 48th hours (p<0.05). Analgesic consumption was significantly lower in GD than in GM at postoperative 24th and 48th hours (p<0.01).
CONCLUSION: Dex combined to epidural anaesthesia ensured adequate sedation as midazolam; moreover dex infusion was associated with lower heart rate, reduced troponin levels and reduced analgesic requirements. These effects persisted even after discontinuation of dex infusion.

İzole Persistan Patent Duktus Arteriyozuslu Pediyatrik Olguda Nonkardiyak Cerrahi İçin Anestezi Yönetimi

Kardiyak patolojileri bulunan çocuklarda nonkardiyak cerrahi için güvenli bir anestezi yöntemini planlarken temel hedef, hastanın preoperatif durumu ve eşlik eden kardiyak problemin fizyolojisine göre optimal oksijenizasyonun sağlanması ve kardiyak stabilitenin korunmasıdır.
Bu sunumuzda izole persistan patent duktus arteriyozusu bulunan pediyatrik olguya uyguladığımız anestezi yöntemini ve bu olgularda güvenli anestezi yönteminin nasıl olması gerektiğini irdelemek istedik.
The main goals for a safe method of anesthesia during noncardiac surgery in pediatric patients with congenital cardiac anomaly are evaluation of patient’s preoperative condition, maintaining optimal oxygenation according to the pathophysiology of accompanying cardiac disease and keeping of cardiac stability.
In this report, we aimed to discuss both the method of anesthesia that we applied to a pediatric case having patent ductus arteriosus and what shoud be done for a safe anesthetic method in patients with patent ductus arteriosus.

Histeroskopi, intrauterin patolojilerin tanı ve tedavisinde; hızlı postoperatif derlenme ve düşük morbidite sağlaması nedeni ile “altın standart” bir teknik olarak kabul edilmektedir. Histeroskopi, güvenli ve kolay uygulanabilen minimal invazif bir yöntem olarak kabul edilmesine rağmen, özellikle operatif histeroskopiler anes¬tezistler açısından dikkat edilmesi gereken birçok riski de beraberinde taşımaktadır. Bu olguda, operatif histeroskopi uygulaması sonrasında venöz hava embolisi gelişimini suna¬rak bu komplikasyonun yönetimine dikkat çekmeyi amaçladık.
Hysteroscopy for diagnosis and treatment of intrauterine pathologies such as rapid postoperative recovery and lower morbidity due to provide the "gold standard" is considered as a technical. Although hysteroscopy is a safe and easy method of minimally invasive procedures used during the procedure, particularly operative hysteroscopy brings along many risks that anesthetists should consider about. In this case, we presented venous air embolism developed after the operative hysteroscopy, and aimed to draw attention to the management of these complications.
